News Across Latin America, January 2020 Issue

We are pleased to share with you the first issue of the year of News Across Latin America, the quarterly publication for the ISPOR Latin America Consortium. We would like to take this opportunity to say thank you for your collaboration and support of ISPOR during 2019. We hope to continue growing with you and bring new and exciting opportunities for our members to increase the understanding and usage of HEOR in the region. We hope you can join us in this effort this 2020. The ISPOR Latin America Consortium will be organizing many more high impact activities including publishing cutting edge webinars and health policy articles, hosting leadership trainings, networking events and organizing initiatives during ISPOR’s conferences throughout the year, along with our regular committee activities and business meetings. We look forward to your excellent participation!

This issue also includes updates from our chapters on health policy and HEOR, and updates from around the Latin America region.

On behalf of the ISPOR Latin America Consortium, we thank you once again for your exceptional contribution during the past year and we look forward to our continued collaboration in advancing HEOR in Latin America this 2020.

Latest HEOR advances in Brazil

2019 was a very dynamic year when we think about HEOR advances and improvements that took place in our country. Concerning our public health care system, on which 75% of the population depends on, changes have been made towards more equitable access (for example, in rare diseases) by prioritizing care in specialized hospitals spread throughout the country This is expected to improve health care spending once this action optimizes resource allocation, and consequently reduce waste. Emicizumab for the treatment of hemophilia, Certolizumab for psoriatic arthritis, Infliximab and Vedolizumab for ulcerative recto-colitis, Abiraterone for prostate cancer, Nusinersena for spinal muscular atrophy and Brentuximab for lymphoma, are examples of new technologies that have been incorporated into SUS and will be available for the entire population. Regarding the private system, the decline in the number of people that have health care insurance has apparently come to an end; which may indicate a reversal of the previous worrisome tendency, but this will be confirmed during the next year. In 2020, CO-Saude, an experts’ committee formed by delegates from several stakeholders (physicians, payers, patient advocacy groups) which is responsible for the process of renewing the Agência Nacional de Saúde’s “ROL”, a list of all procedures with mandatory coverage for all health care plans, will present their decision about more than 500 new technologies that have been presented as candidates to join this list. The whole mechanism of application of new technologies has been redefined by the government’s agency, now legitimating public participation; what was considered huge progress – n the past only a limited number of players could take part in the process and present their demands. New improvements are hopefully waited for the next year.

¿Cuál es el rumbo del sector salud según el plan nacional de desarrollo? pacto por Colombia, pacto por la equidad

El PND «Pacto por Colombia. Pacto por la equidad» está conformado por 25 pactos que se construyeron a partir de la llamada “ecuación de bienestar”, según la cual

Legalidad + Emprendimiento = Equidad

El pacto central es el de la legalidad, que tiene como bases la seguridad y la justicia (entendidas como la “consolidación del Estado Social de Derecho, para garantizar la convivencia y asegurar el acceso a una justicia eficaz, eficiente y efectiva”, PND p. xxxvi), pues en ella se cimientan bases de los demás pactos. En particular, la legalidad es crucial para generar emprendimiento productivo, el que a su vez es fundamento para mayor crecimiento económico y, por esa vía, de incremento en la inclusión productiva. Esto implica, asegura el PND, mejoras en la equidad, entendida como la garantía de la igualdad de oportunidades para todos los ciudadanos. No son claros en el PND los mecanismos específicos por medio de los cuales tienen lugar los eslabonamientos de los distintos componentes de la “ecuación” ni de la secuencia de efectos, la magnitud y sinergia de los mismos y los escollos para lograrlos, aunque se postula que de implementarse las medidas propuestas en el plan el país tendrá una senda de crecimiento potencial de 4,5% anual, sustentada en un incremento del 100% en el crecimiento anual de la productividad factorial al final del periodo de gobierno (DNP, en https://www.dnp.gov.co/Paginas/Gobierno-presenta-al-Congreso-Pacto-por-Colombia-pacto-por-la-equidad.aspx).

Además de la equidad que resulta del pacto por la legalidad, se formula un pacto por la equidad, descrito como uno “por una política social moderna centrada en la familia, eficiente, de calidad y conectada a mercados, que busca garantizar igualdad de oportunidades de inclusión social y productiva para todos” (PND, p. 180), cuyo objetivo es acelerar el cambio social en el país. En este pacto se inscribe la estrategia y las políticas públicas en para el sistema de salud: «Salud para todos con calidad y eficiencia, sostenible por todos». La estrategia tiene dos componentes:

  1. Diagnóstico en seis áreas: rectoría del sistema de salud, salud pública y resultados en salud, articulación dentro del aseguramiento, retos en la prestación de servicios para mejorar la experiencia de los usuarios, gestión del talento humano y eficiencia y gestión de recursos.
  2. Seis objetivos orientadores y sus correspondientes estrategias de política en las áreas definidas en el primer punto, los que a su vez se articulan con la Agenda 2030 y los Objetivos de Desarrollo Sostenible.

Presidente chileno lanza reforma al fondo de salud

El presidente chileno, Sebastián Piñera, lanzó un proyecto de ley para modificar el Fondo Nacional de Salud (FONASA) en base a “lo que la gente siente”. El presidente comunico que el objetivo es tener un plan de salud universal de calidad con tiempos de espera máximos garantizados para recibir atención médica, baja en el precio de los medicamentos y un FONASA que será defensor de estos pilares de esta nueva reforma, según Piñera, el plan permitirá ampliar las licitaciones públicas para que clínicas externas a FONASA brinden asistencia a los usuarios del sistema público. Además, se extenderá la participación estatal en la compra de medicamentos, de llevarse en acción, este plan ayudará a más de 14, 5 millones de usuarios a enfrentar los altos costos de las enfermedades graves presentes en el país.

Claves del nuevo Plan Nacional de Salud en México

 México está en el proceso de implementar una reforma de salud que se basa en los siguientes principios y avances:

  • It will be a model of universal and free access, which will offer comprehensive care.
  • By 2020, the National Institute of Health for Welfare will guarantee the supply of medicines and healing material, for which, a strategy based on three pillars will be implemented: public procurement, supply planning and distribution.
  • Specialized doctors and nurses will be guaranteed in each hospital. For this, the Ministry of Health will issue a call for health professionals to join the farthest medical units, but in which they will be offered better salaries.
  • Se preverá el mantenimiento y construcción de más hospitales. Aún hay 236 hospitales sin concluir, por lo cual se invertirán 5,359 mdp para terminarlos, además de 6,292 para adquirir equipo hospitalario. Pero el plan Nacional de Infraestructura asciende a 34, 254 millones de pesos para ampliación, construcción y mantenimiento de centros de salud, hospitales de segundo nivel y de alta especialidad.
  • Se regularán a 86, 268 trabajadores eventuales y por honorarios del sector salud.

Últimos avances en Perú:

En recientes noticias destacadas en Perú. También, el Dr. De la Cruz estará participando como experto invitado por el Ministerio de Salud para discutir el lanzamiento del: “Manual para Evaluaciones Económicas en el Perú.”

En otras actividades del capítulo, se encuentra en proceso de desarrollo el CURSO INTERNACIONAL DE ETS en Lima, Perú. Mas de 100 asistentes, representantes del sector público y privado, ex Ministros de Salud estarán presentes, así como organizaciones civiles y de pacientes.

Adicional, para cerrar el 2019, por primera vez en el capítulo ISPOR Perú se convocaron a todos los Expresidentes a un evento especial con la presencia de los afiliados e interesados, donde se les entregó un reconocimiento y una medalla para reconocer su servicio.

Algunos de los objetivos del capítulo para este 2021 serán:

1. Fortalecer el capítulo ISPOR-PERU con actividades mensuales

2. Expandir la membresía, incluyendo a las provincias del interior del país.

3. Creación del Capítulo de Estudiantes

4. Trabajar estratégicamente con la Academia y Universidades

5. Organizar cursos y eventos para los miembros del capítulo en el 2020.

6. Participar de los eventos regionales y el evento de ISPOR 2020 en Orlando.

  • Preparing for 2020: We encourage you to submit your Chapter Annual Reports. The ISPOR Chapter Annual Report submission deadline is January 31, 2020. Your achievements are important to us and to the region. Chapters that want to be considered for the Outstanding Chapter Award should submit fully complete annual reports by the deadline for qualification. Chapters that don’t submit their reports may risk losing their chapter status.
  • ISPOR Outstanding Regional Chapter Award – recognizes outstanding contributions and leadership of ISPOR Regional Chapters in advancing the ISPOR mission in ISPOR global regions. The Award is offered in three categories based on Chapter membership size: small, medium, large. ISPOR staff assess the Chapters contributions by applying eligibility and selection criteria in their review of the information available from the Chapter Annual Reports, Chapter input to the ISPOR publications and ISPOR activities throughout the year.

Prior to his appointment as editor-in-chief for ViHRI he was serving as an associate editor for Value in Health, the Society’s flagship journal.
